Introduction

It can be difficult for a participant, family and/or carer to find the right type of support and to coordinate and access a variety of disability services. Specialist Support Coordination NDIS aims to support individuals with complex support needs to understand their choices, access appropriate supports and ensure that they are making the best use of their plan. Specialist Disability Housing (SDH) is a type of support coordination that is designed to meet the individual needs, goals, preferences and support needs of people.

1. What is Specialist Support Coordination NDIS?

The higher level of Specialist Support Coordination is designed for participants with more complex situations, or who need extra support to implement their NDIS plans. It can assist service participants in navigating through the service system, dealing with service challenges, establishing a relationship with service providers, and becoming more independent.

Support co-ordination is personalised at Specialist Disability Housing. Rather than a 'one size fits all' approach, it is about the individual; understanding their situation and supporting them to make choices about their supports.

2. Individualised NDIS Support Planning

Each participant has his or her own goals, context and needs for support. The first step to effective co-ordination is to know what the person cares most about. Through Specialist Disability Housing, the specialist will provide support to participants to help them identify priorities and plan for how they can access the right supports in practice.

Support coordination can include information to an individual on their NDIS plan, connecting them with service providers, coordinating services to the plan and assisting to remove some of the barriers that may impact on their ability to achieve their goals. Enhanced communication and supervision can help make the NDIS more manageable.
