Step 1: Gather and Prepare the US Passport Application Document

1. Replace Lost Passport: Completed Application DS-11 form online

Please check the link at the lowest of this unit to complete and make your passport application DS-11 online. After you fill out the form online, you will see at the lowest of the page a key that says Generate Form. Please click the Make Form button, and your application form DS-11 will appear completed on your screen. Please note, the passport activity doesn’t receive a handwritten application form; all applications have to be submitted online.

• Please copy two copies of the completed application form DS-11.
• Do not copy the form on double-sided pages.
• Do not sign this application form till you are taught to through Authorization.
• Afterward, fill out and manufacture the form, and close the window to return to your passport orders.

2. Completed DS-64 form Statement regarding lost or stolen passport

3. Proof of US Citizenship: one of the following documents

• An expert US birth certificate
• A perished US passport
• Inventive naturalization certificate

4. Two identical passport photographs

Booked within the past 6 months, showing current arrival. Must be an authorized passport-type only and around 2 x 2, with a strong background. The photo must be in color. Do not wear a hat, glasses, or any item that shelters the face. Face onward only; no side shots or profiles will be accepted. Home-based digital Photographs are not satisfactory. You may income your passport photos at Walmart, Walgreens, or CVS.

5. Proof of Identification

• State-issued driver’s license at least 6 months old manner a photo
• Armed ID bearing a photo

6. Replace Lost Passport: Proof of Departure

Please deliver one of the following items if you have one:

• A copy of your route from the travel agent or airline, or

8. Processing Time and Fees

A. US Government Fee:
Make one check or money order owed to the US Department of State. This is the government dispensation fee.

• Passport Book, $212.05
• Passport Book and Passport Card: $242.05
• Write the applicant’s full name and date of birth on the check or money order.
• All private checks must show a printed address.
• Do not cross out and revise any data on the check.
• Provisional or starter check is not acceptable.

Make the second check or money order in the amount of $35 owed to the Post Office or the Court that you visit.

9. Replace Lost Passport: Order Form

Click here to totally fill out the order form. You will be asked to input your private information, order information, and shipping information. After entering your data, click on Submit Order, which will create your finished order form, then copy it out and sign it.

Step 2: Get Your Application Authorized

All first-time or new applicants are required to have their applications legalized by local government officials called Receipt Agents. Take all of the documents listed above to a  Passport Receipt Ability and notify the agent that you are using an accelerated service to hand-carry your application.
